PHYSIOLOOPS, LLC
CONCIERGE HEALTH COACHING POLICY & AGREEMENT
This Concierge Health Coaching Agreement ("Agreement") is entered into by and between Physioloops, LLC, located at 15B Brookside Heights, Wanaque, NJ 07465 ("Physioloops"), and the individual receiving services ("Client").
By enrolling in the Physioloops Concierge Health Coaching Program, the Client acknowledges and agrees to the terms outlined herein.
1. NATURE AND SCOPE OF SERVICES
Physioloops provides concierge health coaching services with a focus on neuroplasticity retraining, musculoskeletal rewiring, cellular health, and wellness optimization. These services include:
  • Lifestyle and movement coaching to enhance functional wellness.
  • Guided neuroplasticity-based exercises to improve movement patterns and coordination.
  • Education on postural alignment, movement efficiency, and self-care strategies.
  • Nutritional and detoxification support for cellular health optimization.
  • Lab testing recommendations to assess functional imbalances, if applicable.
Not Medical or Physical Therapy Services:
This program does not constitute medical treatment, physical therapy, rehabilitation, or a substitute for professional medical advice. Participation does not establish a patient-provider relationship between the Client and Physioloops or its representatives. Physioloops does not diagnose, treat, or manage any medical conditions.
The Concierge Health Coaching Program is a wellness and fitness-focused program designed for individuals seeking to optimize their movement, posture, and overall functional well-being.
2. CLIENT RESPONSIBILITIES
The Client acknowledges and agrees to:
  • Disclose all relevant health information to Physioloops prior to participation.
  • Consult a licensed medical professional regarding any underlying medical conditions before engaging in wellness coaching.
  • Follow the recommended wellness strategies provided by Physioloops.
  • Assume full responsibility for their own health decisions and participation in the program.
3. LAB TESTING POLICY
If lab testing is recommended, the Client understands and agrees to the following terms:
  • The Client will be provided with detailed information on recommended tests.
  • Lab testing is not covered by insurance, as it is not used for medical diagnosis or treatment.
  • Tests are optional and provided for educational purposes only, offering insights into nutrient deficiencies, toxicities, and cellular health.
  • The Client is responsible for the full cost of lab testing, which must be paid directly to the testing provider.
A Report of Findings Consultation ("ROFC") will take place once all lab results are received, typically within three weeks. The ROFC may exceed one hour depending on the complexity of the tests. The Client is encouraged to have a spouse or support person present for lifestyle planning. A digital copy of lab results will be provided to the Client following the consultation.
4. SUPPLEMENT RECOMMENDATIONS
The Client acknowledges and agrees that:
  • Supplements may be suggested to address identified nutrient imbalances or support detoxification.
  • The goal is to reduce reliance on supplements over time as the Client improves their health through lifestyle modifications.
  • The Client is not required to purchase supplements and may seek alternatives.
  • Supplements are not intended to diagnose, treat, cure, or prevent any disease and should be discussed with the Client’s healthcare provider prior to use.
5. NEUROPLASTICITY AND MUSCULOSKELETAL REWIRING – FITNESS AND WELLNESS TRAINING
The Client acknowledges that neuroplasticity retraining and musculoskeletal rewiring offered by Physioloops are part of a fitness and wellness program that aims to:
  • Promote optimal movement patterns and postural alignment.
  • Reduce compensatory movement habits that contribute to tension or inefficiency.
  • Support brain-body retraining through structured exercises.
These services are not a substitute for physical therapy, chiropractic care, or medical intervention. No diagnoses, manual therapy, or medical treatments are provided.
The program is educational and self-guided, empowering Clients to implement strategies for better movement and function within their daily lives.
6. PAYMENT TERMS AND CANCELLATION POLICY
The Concierge Health Coaching Program is an ongoing premium wellness service that operates on a customized, flexible schedule.
Fees and Payment:
  • The Client agrees to pay the concierge coaching fee as outlined in their individual agreement.
  • Payment is due before the commencement of services.
  • No insurance reimbursements will be processed, as these services do not constitute medical care.
Cancellation and Rescheduling:
  • Clients may cancel or reschedule sessions with at least forty-eight (48) hours' notice to avoid forfeiture of the session.
  • Missed appointments or cancellations within forty-eight (48) hours will not be refunded or rescheduled.
Refund Policy:
  • Payments for concierge coaching services are non-refundable.
  • No partial refunds or credits will be issued for unused sessions.
Subscription and Retainer Fees:
  • If applicable, Clients enrolled in a subscription or retainer-based model agree to automatic billing for continued access to services.
  • Clients may terminate their membership by providing written notice at least fourteen (14) days prior to the next billing cycle.
7. WAIVER OF LIABILITY
By participating in the Concierge Health Coaching Program, the Client acknowledges and assumes all risks associated with physical movement and wellness activities. The Client expressly waives and releases Physioloops, LLC, its representatives, employees, contractors, and affiliates from any and all claims, liabilities, damages, or causes of action arising from participation in the program, including but not limited to injuries, discomfort, or adverse health effects.
Physioloops is not responsible for medical complications, injuries, or exacerbation of existing conditions. Clients are advised to seek medical clearance before engaging in the program.
8. PRIVACY AND COMMUNICATION CONSENT
  • All Client information is handled in accordance with Physioloops’ Privacy Policy.
  • Communication services are provided with the Client’s consent, subject to privacy limitations and potential technical disruptions.
9. DISPUTE RESOLUTION AND ARBITRATION
  • Any disputes arising from this Agreement shall be resolved through binding arbitration in the State of New Jersey.
  • Clients waive their right to initiate lawsuits, class actions, or jury trials.
  • The prevailing party in arbitration shall be entitled to reasonable attorneys’ fees and costs.
10. TERMINATION OF SERVICES
Physioloops reserves the right to terminate services under the following conditions:
  • The Client violates any provision of this Agreement.
  • The Client engages in disruptive, unethical, or inappropriate behavior.
  • The Client fails to comply with payment obligations.
Upon termination, the Client shall lose access to all coaching materials and services.
11. GOVERNING LAW
This Agreement shall be governed by and construed in accordance with the laws of the State of New Jersey. Any legal proceedings shall be conducted exclusively in the courts of New Jersey.
12. ACKNOWLEDGEMENT AND ACCEPTANCE
By signing below or accepting electronically, the Client acknowledges that they have read, understood, and agreed to the terms of this Agreement. The Client further acknowledges that this program is not physical therapy, medical treatment, or a substitute for healthcare and assumes full responsibility for their participation.
